A Living with Angels Volunteer will contact you by phone to make an appointment.

To get reimbursed on past expenses (Not Older Than 30 Days)
You will need to take the following to your appointment:
     1. Valid I.D. of Parent or Legal Guardian
     2. Proof of residence with your name. Examples: Light Bill or Water Bill
     3. Your child's birth certificate.
     4. Your child's hospital admission and release papers.
     5. All receipts that you would like us to help you with. ( Food, Gas, Hotel, Etc.. )
6. Maximum of $300 per calendar month


If your child has a future appointment and you need assistance prior to traveling
You will need to take the following to your appointment:
     1. Valid I.D. of Parent or Legal Guardian
     2. Proof of residence with your name. Examples: Light Bill or Water Bill
     3. Your child's birth certificate
     4. Your child's local Doctor confirmation of estimated hospital stay with appointment time and date
5. Maximum of $300 per calendar month

To receive a $20.00 H.E.B. gift card
     We may be able to meet with you at your Doctors Clinic ( Depending on location and volunteer availability )
     1. Valid I.D. of Parent or Legal Guardian
     2. Signed confirmation that you received the gift card with $ amount
     3. Minimum 5 day notice of your child's appointment
     4. Your child's Doctor name and Clinic address
     5. Time of your appointment
6. Maximum of One Gift Card per week


Child must be 17 years old or younger to qualify for assistance
